HealthFirst Family Care Center, Inc., is a Federally Qualified Health Center (FQHC), offering services in offices located in Laconia, Franklin, and Canaan, NH and we proudly serve 28 rural townships: Alexandria, Andover, Ashland, Belmont, Bridgewater, Bristol, Canaan, Center Harbor, Danbury, Dorchester, Enfield, Franklin, Gilford, Gilmanton, Grafton, Groton, Hebron, Hill, Laconia, Meredith, Moultonborough, New Hampton, Northfield, Orange, Salisbury, Sanbornton, Sandwich, and Tilton. HealthFirst is a Federally Qualified Health Center and therefore we receive partial program funding from Health and Human Services, Bureau of Primary Health Care Services. This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Harbor Care is an innovative network of nonprofit health care, housing and human services helping adults, children, and families of New Hampshire find solutions to many of life’s most challenging issues. Greater Portsmouth Youth Wellness Coalition (GPYWC) is sponsored by a Drug Free Communities grant. GPYWC involves over 30 members, including youth, parents, and professionals from all sectors across the community. Their monthly in-person meetings, intentional relationship building activities, and dedicated networking opens dialogue, creates connections, and minimizes duplication of efforts. This coalition addresses substance use and mental health challenges among the youth in the Greater Portsmouth region. GPYWC also engages in youth prevention and early intervention programming with a focus on reducing disparities and building positive social and decision-making capacities in youth.

We have decades of experience helping all 50 states and 10 federal jurisdictions enhance well-being and improve health outcomes. We’ve led large technical assistance contracts for the Substance Abuse and Mental Health Services Administration (SAMHSA) to address nationwide problems. And we’ve designed individual projects for states and communities to tackle local issues. Our team includes national subject matter experts and research organizations with expertise in all areas of public and behavioral health.
